INJURIES/ACCIDENTS
If an accident or incident occurs at the club involving a club member, employee or guest:
- Administer First Aid or call an ambulance if necessary
- Keep the injured person calm
- Determine if there were any witnesses to the accident/incident
The League Coordinator must file a Member/Guest Accident Report form with the Club President and SPFAS Administration Manager
GAME SCHEDULE CHANGE
League Coordinators may make changes to game schedules by written request to the Facility Operator, using the Game Schedule Change Request Form. The form must be submitted to [email protected] with a copy to [email protected]. For an initial check about ice availability, you can see check the SPFAS website here.
SPONSORSHIP
The Club may offer support for members to compete in Local, Provincial, National, or International Events. To qualify for funding, members must be in good standing with the Club. Events which do not require qualification through a preliminary or qualifying event will generally not qualify for support from the Club.
The Board of Directors must approve all sponsorship requests, and members who are interested must submit a completed Sponsorship Request Form. Guidelines for the level of support are detailed in theGovernance and Operating Guidelines.
AGM PROXY FORM
Each Registered member in good standing shall be entitled to one vote, or one vote by proxy, on any motion. If you are unable to attend the Annual General Meeting, you may nominate a proxy by submitting a signed copy of the Proxy Form to [email protected] or in hardcopy at the AGM.
